At Your First Appointment
1. Please arrive 10 minutes early to complete the New Patient Form. You can also download the form, complete it and bring it in with you if your prefer.

2. Bring in all your insurance information and benefit booklet (if available) so we can assist you with your dental benefits.
3. If you do not have a dental plan, we offer “CareCredit” and “Dento Plan” to make payment arrangements more comfortable for you where you can enjoy interest free payments for one full year. We also offer budget plans for limited income people.
